1. Assumption of Risk

I, the undersigned, understand that participation in Brazilian Jiu Jitsu and related martial arts activities involves inherent risks of serious injury, including but not limited to sprains, strains, broken bones, head injuries, paralysis, and even death. I voluntarily choose to participate with full knowledge of these risks.

2. Release of Liability

In consideration for being allowed to participate in training, classes, open mats, events, or any activities held at Braman Jiu Jitsu LLC, I hereby release, discharge, and hold harmless Braman Jiu Jitsu LLC, its owners, instructors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action related to any injury, illness, damage, or death arising out of my participation.

3. Medical Clearance

I certify that I am physically fit to participate in martial arts training and that I have no medical condition that would prevent my safe participation. I understand that it is my responsibility to consult with a physician prior to engaging in training.

4. Consent to Treatment

In case of injury or medical emergency, I authorize Braman Jiu Jitsu staff or representatives to obtain emergency medical treatment on my behalf. I accept financial responsibility for any medical care provided.

5. Media Release

I grant permission for Braman Jiu Jitsu to take and use photographs, videos, or other media of me for promotional and marketing purposes without compensation.

6. Parental Consent (if under 18)

I, the parent/legal guardian of the minor named above, consent to their participation and agree to all terms of this waiver and release on their behalf.

7. Governing Law

This agreement shall be governed by the laws of the State of Rhode Island. If any portion is found invalid, the remainder shall continue in full effect
